First hybrid cardiac surgery made in Almaty

Kazakhstan surgeons have made their first hybrid cardiac surgery on abdominal aortic aneurysm earlier this month in Almaty. The results were presented two weeks after the surgery in the Central Clinical Hospital in Almaty, Tengrinews reports.

A hybrid cardiac surgical procedure in a narrow sense is defined as a procedure that combines a conventional surgical part with an interventional part, using some sort of catheter-based procedure guided by fluoroscopy imaging in a hybrid OR without interruption.

Eduard Sin, 52, has been suffering from the chronic disease for about 10 years. "I had short breath, had to follow a strict diet and limit activities. And I had persistent pains. I took a computed tomography because of the pains and it revealed the abdominal aortic aneurysm. And now, 10 days after surgery, I feel good," Eduard Sin said.

According to the doctors, they used a stent graft in this surgery.

A stent graft is a tubular device composed of special fabric supported by a rigid structure, the stent, which is usually metal, are used mainly for vascular interventions. The stent graft serves to seal tightly and reinforce a weak spot inside the artery, saving it form rupture.

To restore the blood flow to the brain and the myocardium the surgeons shunted the brachiocephalic trunk, left carotid artery and right coronary artery.

“To put it simple, the stent consists of a stainless steel mesh liner that is inserted through the femoral artery with a thin catheter that delivers the vascular implant to the bulging aneurysm. The operation on the abdominal aortic aneurysm lasted for 3.5 hours. Six surgeons took part in the surgery. The patient feels good, his condition is stable. He will be discharged from the hospital on December 30,” endovascular surgeon Sergei Musayev said.

According to the doctors, open surgery to repair abdominal aortic aneurysm that involved replacement of the damaged part of the aorta with an implant, was the only way to treat aneurysm patients in Kazakhstan five years ago. This was a very invasive procedure that carried very high risks and involved long intensive care.

The new way of treatment - the endovascular repair - is equally effective, but the patients recover much faster and the whole process is much easier, that is why the treatment can be applied to 60% cases or more.

Abdominal aortic aneurysm, also known as ‘AAA’ is a localized dilatation (ballooning) of the abdominal aorta exceeding the normal diameter by more than 50 percent, and is the most common form of aortic aneurysm.

A team of leading surgeons of the Central Clinical Hospital of the Medical Center of Kazakhstan President's Administration lead by an invited expert from Lithuania -- professor Almantas Maleckas -- made a bariatric surgery a little over one month ago. It is a new surgery for Kazakhstan's weight loss practices. The surgery involved re-routing the small intestines to a small stomach pouch (gastric bypass).

The patient was a 47-y.o. woman from Almaty. She has been overweight for a long time. The woman wasn't tall, but her weight was 150 kilograms. The obesity was causing her different health problems: arthronosos, diabetes, etc. Other ways of treatment didn’t suit her, and the diets had only a temporary effect. That is why the woman decided to take a weight loss surgery.
